Sonali Ojha on Mental Health as Everyday Health in India

From Stigma to Strength: Accepting Mental Health as Everyday Health

Sonali Ojha

Research Clinical Psychologist

“Can any one of us honestly explain why most of our largest hospitals still do not have designated mental health wards? Is having only a psychiatrist or an on-call psychologist enough to address one of the fastest-growing health challenges of our time?”

Recently, I had the opportunity to visit one of the fastest-growing medical hubs in West Bengal. During my interaction with hospital administrators and healthcare professionals, I raised this very question that- Why are dedicated mental health wards still absent in many large multispecialty hospitals?

The response was honest and revealing.

“It requires specialised manpower, trained professionals and multidisciplinary skills that we currently lack. We have also noticed that mental health patients and their caregivers are reluctant to visit crowded and busy hospitals. Thus in long run we see that there is no point in investing”

Their answer reflects a reality that extends far beyond a single hospital. India does not merely face a shortage of infrastructure, it faces a shortage of trained mental health professionals, psychiatric nurses, clinical psychologists, psychiatric social workers, occupational therapists, and integrated care systems capable of delivering comprehensive mental healthcare. Perceived stigma remains a significant barrier, discouraging many individuals from accessing mental health services in hospital settings despite the need for professional care. Ironically, while society encourages conversations about mental health, many people still hesitate to walk into a hospital seeking psychological care because they fear being judged more than being treated. Building a mental health ward is not simply about allocating physical space, it requires sustained investment in people, training, interdisciplinary collaboration, reducing stigma through awareness and long-term commitment.

Yet this explanation also raises an important question. If mental illnesses contribute significantly to the nation’s disease burden and affect millions of Indians across all age groups, can we continue to view specialised mental healthcare as a service that can wait? We would never postpone establishing cardiac units because cardiologists are scarce, nor would we ignore oncology services because cancer specialists are limited. Instead, we invest in training, infrastructure, and systems. Mental healthcare deserves the same vision and urgency.

This is where India must move beyond awareness. While campaigns have successfully encouraged people to speak about anxiety, depression, stress, and burnout, acceptance requires structural change. It means designing hospitals where mental health is integrated into mainstream healthcare rather than remaining an isolated specialty consulted only when a crisis arises.

The journey from stigma to strength begins not with slogans but with systems. It begins when we recognise that healing the body without caring for the mind leaves recovery incomplete. It begins when policymakers, healthcare institutions, and society collectively accept that mental health is not separate from health, it is everyday health.

These are uncomfortable questions, yet they deserve honest answers. When a patient arrives at a hospital with chest pain, there is a cardiac unit. When someone suffers a stroke, there is a neurology department. A child with severe respiratory distress is immediately shifted to a specialised intensive care unit. But when a person experiences a severe panic attack, suicidal thoughts, acute depression, or psychological trauma, the response is often fragmented. They may be referred to a psychiatrist, asked to give time for self assessment by elders, advised to return later for counselling, or transferred to another facility. Why do we continue to separate the mind from the body when scientific researches has repeatedly shown that they are inseparable?

India has made remarkable progress in bringing mental health into public conversation. Schools organise awareness programmes, workplaces celebrate Mental Health Day, and social media is filled with discussions about anxiety, burnout, and depression. Awareness has increased significantly. However, awareness without structural acceptance remains incomplete. True acceptance is reflected not only in conversations but also in how hospitals are designed, how healthcare professionals are trained, and how society responds when someone seeks psychological help.

Mental disorders affecting the brain or the mind deserve the same urgency, infrastructure, and dignity as any other medical condition. But mental healthcare is still treated as an optional extension of medicine rather than an essential component of it. This silent separation reinforces the outdated belief that emotional suffering is somehow less real than physical illness.

The consequences are visible every day. Patients recovering from cancer struggle with depression. Individuals admitted after heart attacks develop severe anxiety about recurrence. Women facing complicated pregnancies experience overwhelming emotional distress. Caregivers silently battle exhaustion and burnout. People living with chronic illnesses often experience hopelessness, loneliness, and reduced quality of life. Their physical treatment may be excellent, but their psychological suffering frequently remains unaddressed. Healing, therefore, remains incomplete.

A dedicated mental health ward is far more than a room with beds. It represents a philosophy of care that recognises psychological well-being as central to recovery. Such units can provide crisis intervention, suicide risk assessment, behavioural management, counselling, family education, and multidisciplinary rehabilitation within the same hospital. They allow psychiatrists, psychologists, psychiatric nurses, social workers, occupational therapists, and physicians to work together rather than in isolation.

Equally important is the integration of mental health screening into routine healthcare. Every patient admitted with a chronic medical condition should also be assessed for emotional distress. Screening for depression, anxiety, sleep disturbances, substance misuse, and caregiver stress should become as routine as measuring blood pressure or checking blood sugar. Early identification not only improves mental well-being but also enhances treatment adherence, reduces hospital readmissions, and improves overall health outcomes.

Acceptance also begins with healthcare professionals themselves. Doctors, nurses, and allied health workers receive extensive training in managing physical illnesses, but many receive limited education in recognising psychological distress. Building compassionate communication skills, reducing stigma within medical settings, and promoting interdisciplinary collaboration are essential steps toward patient-centred care.

Beyond hospitals, acceptance must extend into our homes, schools, workplaces, and communities. Families should understand that seeking therapy is not a sign of weakness but an act of responsibility. Schools should teach emotional literacy alongside mathematics and science. Employers should build psychologically safe workplaces where employees can seek help without fear of discrimination. Communities should replace judgment with empathy and silence with support.

India’s future mental health strategy cannot rely solely on increasing the number of psychiatrists or psychologists. It must strengthen systems. Every tertiary hospital should strive to integrate comprehensive mental healthcare into its services. Every district hospital should ensure timely psychological assessment and crisis intervention. Every healthcare institution should recognise that treating illness without addressing emotional well-being leaves recovery unfinished.

The shift we need is not merely administrative; it is cultural. For generations, mental illness has been hidden behind labels of “weakness,” “attention-seeking,” or “family shame.” These misconceptions have delayed treatment, damaged relationships, and, in many cases, cost lives. Acceptance means recognising that emotional pain deserves the same compassion and clinical attention as physical pain.

The journey from stigma to strength will not be measured by the number of awareness campaigns we conduct but by the systems we build and the attitudes we change. When hospitals begin treating mental health as everyday health, when every patient receives holistic care for both body and mind, and when seeking psychological support becomes as ordinary as visiting a physician, India will have taken its most important step toward a healthier future.

Mental health does not belong on the margins of medicine. It belongs at its very heart. Only then will we truly move from stigma to strength, creating a healthcare system and a society that heals the whole person, not just the illness.

Thank you.
